Don’t Forget to Utilize the New and Improved Membership Documents and Forms

Important SMACNA membership documents such as the through-chapter membership application and termination form have been redesigned to make chapter administration easier and more consistent. Ensure you are using the latest versions for all future submissions.

All essential membership documents and forms were updated and simplified in 2025. Chapters are encouraged to adopt our new and improved documents for all membership and administrative needs if they have not already. New documents can be viewed and downloaded from the SMACNA website. Go to the Chapter Executive Resources area, then scroll to the Membership Documents section.

The termination form is now a fillable online form! No need to download, print, or scan. Just submit the easy online form. The Membership team will automatically be notified, and your termination request will be processed within 1-2 business days.

Pro Tip: If you have embedded any previous versions of the membership application on your chapter website, please ensure it is replaced with the newest application.
